A Step-by-Step Walkthrough of the D'Viajeros Form

Filling out the D'Viajeros form is one of the last things standing between you and your trip to Cuba — and it's much simpler than it looks once you know what's coming. Here's what to expect, step by step.

1. Start the form within the right window before your flight

The form is completed online and generates a QR code you'll need at the airport, so don't fill it out too early or leave it until the last minute — check the current recommended timeframe before you begin.

2. Personal and travel details

You'll enter your full name exactly as it appears on your passport, passport number, your visa or tourist card number, nationality, date of birth, and flight information (airline, flight number, arrival date). Small mismatches here are the most common cause of delays, so double-check every field against your passport before submitting.

3. Accommodation information

You'll need the address of where you're staying in Cuba for at least your first night — a hotel name and address, or the address of a casa particular. Have this ready before you start the form so you're not searching for it mid-submission.

4. Customs declaration

This section asks whether you're bringing in certain categories of goods (cash over a certain amount, commercial merchandise, etc.). Answer honestly — this isn't the place to guess.

5. Health declaration

6. Review and submit

Once submitted, you'll receive a QR code by email or in the app. Save it, screenshot it, and print it if you can — you'll be asked to show it at check-in and again on arrival.
