Before you start: what to have ready

Gathering these first turns the form into a copy-and-check exercise instead of a scramble for information.

Your passport, open to the photo page
Your visa or tourist card number, if one applies to you — it gets entered into the form itself
Flight number and arrival date for your inbound flight
The full address of your hotel, casa particular or host in Cuba
Contact details: email address and a phone number
A rough list of what you're carrying, especially electronics, medicine or gifts
One form per traveler, including children — have every passport to hand

The five steps

Prepare your details
Passport number and expiry, your visa or tourist card number, flight number, arrival date, and the address of your hotel, casa particular or host in Cuba.
Copy details straight from the documents rather than from memory — that alone prevents most errors.

Open the form within 7 days of arrival
Timing matters. Submitting inside the 7-day window keeps your declaration valid for your trip.
Too early and it may not be accepted; the day before you fly is a comfortable moment to complete it.

Complete the three sections
Immigration, customs and health. Answer honestly and take your time — accuracy matters more than speed.
Immigration covers who you are and where you'll stay, customs covers what you're bringing, health covers recent travel and symptoms.

Review and submit
Check names and passport numbers character by character. Small typos are the most common cause of problems at the border.
Names must match your passport exactly, including middle names and both surnames if you have them.

Save your QR code
Screenshot it, save the PDF and print a copy if you can. Airport connectivity isn't guaranteed, so don't rely on email alone.
Keep the screenshot in a favourites album and the printout in your passport, so it's ready without a signal.

Common mistakes to avoid

Filling it in too early

Submitting outside the 7-day window means the declaration may no longer be valid when you land.

Starting without your visa or tourist card

The form asks for your visa or tourist card number, so get it issued first — otherwise you'll have to stop halfway and come back.

Mistyping the passport number

A single wrong character can stop your QR code matching your document at the counter.

Only one form for the family

Every traveler needs their own declaration and their own QR code, children included.

Relying on airport wi-fi

If your QR code lives only in your inbox, you may not be able to open it when you're asked for it.

What happens when you arrive

At the airport you present the QR code with your passport. An officer scans it, your declaration appears on their screen, and you continue through immigration and customs as normal. There is no paper card to complete on the plane and nothing to hand over beyond the code you already have.

Frequently asked questions

Does it cost anything?: The official declaration itself is free. Assisted services, like the one linked on this site, charge for help completing and checking it.
What if my flight changes?: If your arrival date or flight number changes, complete a new declaration with the correct details and travel with the newest QR code.
Do children need their own form?: Yes. Each traveler needs a separate declaration, completed by a parent or guardian for minors.
Can I complete it from my phone?: Yes, the form works on a phone. Just make sure you can save or screenshot the QR code before closing it.
